Definition of Medical Terms:

breech - There are different types of breech position. What Inuyasha’s pup was presented was footling breech. It means one or both of the baby’s feet are pointing down and the legs will come out first.

vertex - The most common and safest position. The baby’s head facing downward and the head will come out first.

pelvic bones or pelvis - The pelvis is the bony structure located at the base of the spine which consist of three separate bones. This is normally a very inflexible joint, but it softens and becomes more flexible during late pregnancy, allowing it to expand during labour for the baby's head to pass through. A female pelvis is also wider and shallower than a male pelvis.

vagina - A narrow and muscular but elastic organ which connects with the uterus. Penis is inserted into the vagina during sexual intercourse. It is also a passageway for sperm to the egg and for menstrual bleeding or a baby to come out. There is no space inside the vagina unless it is stretched open during intercourse or childbirth.

umbilical cord - A narrow, tube-like structure that connects the developing baby to the placenta. It delivers the nutrients and oxygen the baby needs for normal growth and development and removes waste products.

uterus - A thick muscular-walled located in the middle of the pelvis, behind the bladder and in front of the rectum. The main function is to sustain a developing fetus. It can be stretch to accommodate a growing fetus. Its highly muscular walls contract during labor to push the baby out through the cervix and the vagina.

cervix - Opening of the uterus and protrudes into the upper end of the vagina. Only sperm can enter and menstrual blood can exit because the channel is very narrow. During labor, the channel widens to let the baby through.

sphincter muscles - A sphincter is a ring-like muscle that normally maintains constriction of a natural body passage or orifice and that relaxes as required by normal physiological functioning.

Chapter 4 - Crowning of the Head

Sesshomaru had considered a few possibilities why the pup was in a breech position. One reason could be that the pup was too early to be born. Another reason would be due to the fact that Inuyasha's narrow pelvis which hindered the pup from turning head down into a vertex presentation. Furthermore, Inuyasha's rectum and vagina was only separated by a thin wall of tissue yet shared the same opening. As a result, both his rectum and vagina were only half the size as compare to normal beings.

That was the only plausible explanation as to why Inuyasha was always writhing and screaming like a virgin regardless of how many times they made love. Never did Sesshomaru dream that the source of their pleasure could become a life threatening issue right now.

Since Inuyasha's vagina and rectum shared the same cavity, there was no way he could be fully dilated. Sesshomaru had expected the premature pup to be smaller thus able to pass through the miniature fissure but judging from the size of the pup's body, it was as big as a full-term pup and the head would not be able to come out without ripping open his mate’s nether region.

Sesshomaru was left with only one solution on how to salvage the situation and that was to turn the pup from breech to vertex position. Given that the head was the biggest part, if it could be the first to emerge, it might be able to ease the rest of the body through the birth canal without crushing the umbilical cord again as the pup might not be able to withstand the lack of oxygen for a second time.

To get that done, Sesshomaru needed to push the pup back up into the uterus. Hopefully, the amniotic fluid that was left would be enough to assist in the turning. He needed his mate's cooperation as he could not do this alone.

"My love, the arms of the pup is stuck," Sesshomaru explained. "Your pelvis is not wide enough to allow the head and both arms to pass through together. Turning the pup around is the only way to save both of you."

"I don't... don't know," Inuyasha struggled with his ragged breathing, "What if this is not the right choice? What if the pup..."

"Then trust me to make the decision for you," Sesshomaru interrupted.

Sesshomaru did not want Inuyasha to scare himself with all the unnecessary scenarios. If this dragged any longer, more fluids would have escape and it would be even harder to turn the pup.

"Don't worry," Sesshomaru comforted as he caressed his mate's cheeks affectionately, "I'm here. Let me do all the work."

After some dithering, Inuyasha finally complied. Sesshomaru gave an encouraging smile and gently eased Inuyasha's head onto the grass with his fingers still inside Inuyasha's opening. He then proceeded to kneel beside his mate.

Sesshomaru gathered the pup's bottom into his coarse palm when the pup gave an impressive kick. Normally, a fetus would have die if they were to survive on petite or no oxygen during that stretch of time, but not for their pup. Sesshomaru could sense that the pup's willpower to be born was strong and there was no doubt that he was going to be a bravura fighter when he grew up.

'You have done well,' Sesshomaru thought as he stroked the tiny back of the pup. 'Now it's time for me to save both you and your father but I need your cooperation.'

Sesshomaru doubted that the pup understood his thoughts but the pup had indeed ceased kicking. He took it as a sign to proceed and began to push the pup in, bit by bit.

Inuyasha tried to hold back a groaned when he felt the pup was being force back into his narrow birth canal. Everything seemed to be progressing well as the small chest vanished into the canal. With another push, the pup's tummy would be next to disappear into Inuyasha's body.

"Agaah...!!" Inuyasha bawled when another contraction hit him and the pup was slipping out of him. Once again, the pup was out at the chest level. They were back to where they had started.

'This is not good,' Sesshomaru deliberated mentally. If every time the pup was about to get back in, a contraction would push it out. Repeating this action would only inflict more pain to his mate and result in nothing. Unless the entire pup could get in before the next contraction began. This left Sesshomaru only about a minute to get everything done at a go. It was going to be extremely painful.

Inuyasha's entire body was dripping with sweat and the grass underneath him was soaking wet. Watching his mate's aggrieved features, Sesshomaru was wavering if he should let Inuyasha take the potion given by Kagura.

"Love," Sesshomaru spoke as he held up the small bottle using his free hand. "This potion may help in relief the pain but it was given by Kagura."

Upon hearing the name, Inuyasha recoiled and pushed the bottle away, "No no, not her again?!"

Strictly speaking, Kagura could be classified as Inuyasha's enemy but she was definitely not a powerful youkai. Besides, Inuyasha never cowered away from strong opponents. There were definitely something more than what it seemed to cause such a reaction from Inuyasha.

"What's the matter?" Sesshomaru asked as he wiped the sweat from Inuyasha's forehead with his sleeve.

Inuyasha hesitated before muttering his response, "Do you remember the time I got your arm back from Naraku?"

Sesshomaru nodded. That incident happened before they become mate. He recalled that day when Inuyasha came to his castle with his arm. Inuyasha looked appallingly distraught and stench was wafting off from his body. There were huge ugly bruises on his face. Inuyahsa's robe was ragged with a torn sleeve which revealed his dislocate shoulder and numerous deep cuts on his torso.

Inuyasha tossed the arm to Sesshomaru and absconded without saying a word. Later that day, Sesshomaru learnt that Naraku had been trying all means to get rid of his human part. In his desperation, he resorted to using potions but need an experimental subject before he consumed it himself.

Inuyasha was not only his mortal enemy but also a hanyou, just like Naraku. It would be killing two birds with one stone if Naraku could expel his human part out and get rid of Inuyasha at the same time.

Naraku knew that Inuyasha felt self-reproached for slicing off his brother's arm thus used it as bait to lure the young hanyou and drugged him, leaving him boneless and vulnerable. After weeks of cruel experiments imposed on Inuyasha, he finally managed to escape, but not without seizing the arm first.

"What does it have to do with Kagura?" Sesshomaru puzzled.

Sesshomaru got his answer immediately when his mate shrunk away at the name. This response could only mean one thing; she was the one responsible for carrying out the experiments.

Horrible scenes flashed through Inuyasha's eyes as he began, "She for...forced the potions down my throat... ruthlessly injecting them into my veins all over the body using... using tentacles. It h...hurts."

Inuyasha was weeping uncontrollably as he could clearly remember the foul tasting potions and the pain it had caused. "That day, she had some cuts on her clothing and bruises on her face. She was enraged and kept whipping and hammering me, yelling how come there was still no result... how useless I am..."

"That's enough, my love." Sesshomaru hugged his mate securely with the free arm. He got a pretty good idea what Inuyasha had gone through and did not want him to dwell too deeply on it.

However, Inuyasha was too caught up into his own thoughts that he did not hear a word or feel his mate's touch and continue his recounting.

"Suddenly, the tentacles held me upside down. It pulled my legs so wide apart that I thought they are going to rip me open. It wouldn't be too bad that way..." Inuyasha was trembling rowdily as he could still sense the feeling of having those serrated tentacles surrounded his body.

"S...she brutally shoved bottles... into me. Bottles after bottles empty their content down my anal passage. I never had anything in there before... It really hurts... I could feel the bottles cut into my inner walls... the tearing around my hole when the tentacles pulled it wider... to accommodate more bottles. The mixture burned inside me... my rectum and organs were on fire. The mixture seeped into the tears and cuts... and came into contact with the raw flesh..."

Sesshomaru knew the experiment that Naraku performed on his mate results in the growth of female reproductive organs inside Inuyasha's body. What he did not aware was how it was being implemented.

"Stop it! You are making yourself miserable," Sesshomaru shouted as the bottle in his palm shattered into pieces. Clenching his hand into a fist, sharp fragments dug into his palm causing them to bleed.

The potion dribbled into the cuts and burned the wound. 'It...it's poison,' Sesshomaru shuddered, not from the injuries but from the dread that he was manipulated and almost killed his mate and his pup with his own hand. 'How DARE her! I'm gonna kill her.'

"I was afraid of the potions and tentacles. I couldn't do anything. She came every day while I was imprisoned..."

Whatever Inuyasha wanted to say was cut off as Sesshomaru crushed his lips onto his mate, silencing him in the progress. Vacant eyes suddenly came to life when he saw a pair of golden orbs gawking down at him. Inuyasha recognized them and relaxed into the invasion. Their tongues met, sliding along as their kiss deepened. Inuyasha wrapped his arms around Sesshomaru's neck, pulling him closer.

"Uu...a...a!!" Inuyasha bit onto Sesshomaru's tongue when another contraction pierced through him. He tried to break away as he could taste the blood from his mate's tongue. He had hurt him.

Sesshomaru sensed the worries from his mate but continued to hold onto Inuyasha firmly, not letting him escaped. Without breaking the kiss, Sesshomaru murmured, "Don't go... I'm all right." Inuyasha could have bit off his entire tongue right now, for all he cared.

Contractions went and came but not once did Sesshomaru break the kiss. He wanted nothing more than to pour all his passion and love into his mate. Inuyasha was in a trance when he related the incident and he did not like it.

Sesshomaru never wanted to see Inuyasha like that again. How he wished he could suture the emotional scar that his mate had suffered. If only it was something that could be seen or touched to know how deep it went.

Inuyasha cracked his eyes open, not realizing he had closed them. He was stunned to see that Sesshomaru was looking intently at him, throughout their kiss. A sharp pain assaulted him and Inuyasha started coughing hysterically.

Sesshomaru finally released his mate. Immediately, Inuyasha was panting for air and coughing frantically at the same time. With each cough, more fluids were rushing out from his opening.

"I'm sorry, I'm sorry," Sesshomaru repeated as he reprimanded himself for lack of restraint and allowed his emotion to be carried away.

"Don't worry," Inuyasha reassured when the contraction had receded, "Just another contraction."

"I need to push the entire pup back into your uterus before the next contraction starts. It’s going to be painful but please bear it for me." Sesshomaru was glad to change the topic and focused the matter on hands.

Inuyasha nodded. Sesshomaru rolled up his sleeves and repositioned himself between his mate's thighs, spreading Inuyasha's legs and bended them up. When the next contraction struck, Sesshomaru gathered the pup's body into his palm. Once the contraction had subsided, Sesshomaru warned his mate before started pushing the pup back into Inuyasha's vagina.

Inuyasha clasped harshly onto the grasses as he braced himself for the pain. He could feel the pup's body sliding back into his narrow birth canal.

Sesshomaru stopped for a moment when he felt some resistance inside Inuyasha's body. If his guess was correct, the shoulders of the pup were now outside the cervix. To make it less painful, he had to shove both the shoulders into the uterus in a single move. Sesshomaru continued exerted pressure to pry the cervix to open out.

"Wait!" Inuyasha yelled as he was unable to suppress it any longer. He could feel the pup's shoulders jabbing at his cervix which was already stretched by the width of the pup's neck.

"I can't," Sesshomaru said in an apologetic tone. Placing another hand under Inuyasha's rear and lifted his bottom off the ground, leaving both of Inuyasha's legs dangling in midair on either side. "It's coming in."

With an animalistic screamed, Inuyasha's cervix was fiercely forced open to allow the pup's shoulders, arms and chest to press in. He jerked from the excruciating pain that wrecked his body but the pain only magnified with each movement. He panted heavily as tears formed in his eyes, spilling down his ashen face.

Before Inuyasha's body had the time to recover from the forceful invasion, five jagged knuckles that were supporting the pup's bottom prodded its way through the constricted fissure until his sphincter muscles crammed tightly around Sesshomaru's thick wrist.

The quiet forest was filled with painful gasps from the panting hanyou lying on the grass. His eyes screwed shut and his face contorted in pain. His voice was hoarse and weak from all the crying and screaming.

Sesshomaru continued pushing until the pup was fully inside the uterus and his hand stopped at the cervix. Presently, more than half of his forearm was inside Inuyasha's vagina and his fingers were practically swimming among the warm fluids.

"Guaaaaaaah!!!" Inuyasha let out a long whimpers and gasps of pain as all his inner muscles clenched rigidly around Sesshomaru's arm that was inside him.

Sesshomaru moaned as the pressure clamped down on his arm was forcing all the blood circulation to stop, causing his entire arm to go numb. Only then Sesshomaru realized how strong and painful the contractions really were.

Inuyasha held his breath and willed his inner muscles not to push. His head was spinning and throbbing. It hurt worse than anything he ever felt. Tears well in his eyes and ran down his face in a heavy flow.

Inuyasha's rear was about two feet off the ground but Sesshomaru continued to lift it higher, hoping to slow down the fluids from leaking out. Sesshomaru had to do the only thing he could think of at that moment to stop the escaped of the fluids totally. No doubts that Inuyasha was not going to like it but he did not have a choice.

Inuyasha could feel Sesshomaru's hand opened out then clenched into a fist. Instead of withdrawing his hand, it continued to penetrate deeper inside him, poking at his cervix. Fear was written all over Inuyasha's face and he prayed hard that Sesshomaru was not going to do what he guessed he would.

"Wha...what the hell are you doing?" Inuyasha barked. Both of his hands grabbed onto Sesshomaru's arm from in-between his thighs to prevent him from pressing on.

"I'm sorry." Without further ado, Sesshomaru thrust his fist into the cervix but stopped at the knuckles. Inuyasha's lungs seized up and he could not breathe or even screamed from the hideous pain.

Instinctively, the cervix clung firmly onto Sesshomaru's fist. He could clearly feel that the muscles at the cervix were more sturdy and brawny as compare to the vagina.

Inuyasha's face twisted with agony and his eyes blurred with fresh tears. Tighten both of his grips into Sesshomaru's arm as his heart was pounding insanely in his ribcage. He tossed his head back and forth, trying to deny the pain.

A whimper escape Sesshomaru's throat as he watched his mate squirmed in pain. He went very still, allowing Inuyasha's body to get used to the penetration. Abruptly, Inuyasha started coughing.

"Inuyasha!" Sesshomaru exclaimed as he gradually lower Inuyasha's rear to the ground. Having the ass hanging in midair leaving only the shoulders and arms to support the body weight was definitely not a comfortable position, especially when the weight of the pup was adding pressure on the lungs making it difficult for the lungs to expand or breathe freely.

Inuyasha's whined when Sesshomaru shifted to his side, with the arm still inside him. "Love, are you all right?"

Sesshomaru was fretted when there was no respond from his mate. Inuyasha had lost consciousness before and was in trance just now. Furthermore, he could feel Inuyasha getting weaker when the grip on his arm a second ago was not as strong as before. Sesshomaru could not help but worried when Inuyasha failed to answer. Sesshomaru reached out to touch his cheeks. "Inuyasha, answer me!"

"Don't touch me!" Inuyasha retorted and flinched away from his touch. He tried to move away but he soon hissed in pain and collapsed back on the ground again.

Taken aback by his mate unexpected reaction, Sesshomaru pulled back his hand, "What's wrong?"

"You know how painful it is! You are not the one going through all this!" Inuyasha growled, "This is entire your fucking faults!!"

Recognizing that Inuyasha was being his emotionally pregnant self, Sesshomaru did not deny the accusation. He had learned that it was best just to let his mate ranted. He did not mind listening to the threats and various ways Inuyasha wanted to castrate him. Inuyasha was back to his normal self, which was the most important thing. Smiling slightly, Sesshomaru respond to the threats with what it sounded like a concurred grunt.

Sesshomaru was glad that Inuyasha did not cringe from his touch on his next attempt. He stroke Inuyasha's head and murmured comforting words to his mate until Inuyasha's whimpering and trembling had ceased completely.

"Feeling better now?" Sesshomaru asked.

"How can I feel better when your fist is still in my cervix?" Inuyasha accused. He was seething as the pain throbbed vehemently.

"I'll take it out once we have turned the pup," Sesshomaru reasoned out as he continued stroking Inuyasha's head, "Can we do it now?"

"Hmph," was all that came from Inuyasha.

"I need to press on your belly to feel the pup's outline before turning," Sesshomaru explained but sensed that Inuyasha's tense at the mentioned of 'press'.

"It may be quite uncomfortable during the turn, but shouldn't be excessively painful." Sesshomaru reassured, "Let me know if it's painful and I'll stop immediately."

"Really?" Inuyasha questioned.

"Yes, my love," Sesshomaru promised and leaned down to kiss lightly on his lips. He knew the previous occurrence had traumatized Inuyasha. Sesshomaru would be as gentle as possible since he did not need to race against the contractions to get it done.

With an arm still inside his mate, Sesshomaru left with only one hand to do the job. He placed his free hand on Inuyasha's abdomen. Sesshomaru kept a look out at his mate's expression while he was locating the position of the pup, to catch any signs of pain.

Sesshomaru was relieved to find out there was adequate amount of amniotic fluid left in the uterus. He was astonished at the amount of fluids that his mate's uterus contained. So much had gushed out over that stretch of time yet there was ample left. Guess he could credit that the cervix was not fully dilated and the head was caught at entrance of the uterus thus slowed down the leakage.

Sesshomaru was about to move the pup up out of the pelvic bones as he pressed onto Inuyasha's lower abdomen. Suddenly, Sesshomaru felt something warm trickling down his arm. Fearing the worse, he chanced a look.

To his shocked, he realized that Inuyasha had urinated on his arm and it was still trickling forth in-between Inuyasha's legs.

"I'm so sorry! I don't mean it!" Inuyasha apologized and covered his face with both his hands in embarrassed, "It just came out when you press down there..."

Sesshomaru was dumbfounded but soon regained his equanimity. It had been more than a day since Inuyasha last released himself in the castle before he sneaked out. It was already a miracle that he was able to hold it for that long.

Hence, it was only natural that he felt the urged to pee when Sesshomaru pressed on his lower abdomen. Sesshomaru should had Inuyasha emptied his bladder before childbirth but things were happening so fast that he overlooked it.

"It's all right," Sesshomaru soothed, "Don't let this trivial matter bother you."

Inuyasha felt so shameful to urinate onto his mate's arm while Sesshomaru was helping him in the middle of birthing their pup. There was no way he could forgive himself for such an uncouth act.

Inuyasha peeped from between his arms but was caught by Sesshomaru. He tried to rind Inuyasha's arms from veiling his face but Inuyasha stubbornly refused. Sighing, Sesshomaru bended down and nipped onto his mate's sensitive furry ear.

"That was erotic," Sesshomaru whispered.

It was not only to alleviate Inuyasha but Sesshomaru had to admit that it did have a certain effect in provoking his sexual arouse. Sesshomaru was neither a sadistic nor someone who enjoyed sick pleasures from his mate, nonetheless that was something they yet to try out.

Inuyasha went stiffed and sulked, "What are you talking about?" He had turned his head away from Sesshomaru, unintentionally exposing his slender neck.

"If you are going to invite me so seductively..." Sesshomaru proceeded to lick Inuyasha's neck while toying with his nipples using his free hand, "do you want me to show you what I mean?"

Inuyasha's heart skipped a beat when the words sank in. He felt his mate's slick tongue caressing his neck and fingers pinching his nipples. Inuyasha's cock twitched and Sesshomaru could feel his inner muscles clenched around his arm.

Glancing up, Sesshomaru found a shocked visage was staring at him. "Baka," Sesshomaru smirked, finally getting his mate's attention.

Once again, Inuyasha's inner muscles scrunched around Sesshomaru's arm and he knew that it was not from the pleasurable excitement but from a contraction. He ruffled Inuyasha's mane and wait till the contraction passed over.

"I'm turning him," Sesshomaru warned when he felt the muscles had relaxed. He then pressed gently onto Inuyasha's swollen belly and shifted the pup up out of the pelvic bones. Steadily, he guided the pup through a forward somersault.

The pup seemed to get the idea and started kicking which helped to complete the turn. Sesshomaru grinned and he was sure that Inuyasha could feel it too. Then, Sesshomaru gently pushed the pup's head into the pelvic bones so that it was now into the head-down position.

True to his words, Inuyasha yet to experience any pain, only minor discomfort. Sesshomaru was about to remove his hand from the abdomen when he felt the pup was turning back. Instantly, he replaced his hand to prevent the pup from moving away.

Inuyasha had very tight abdominal muscles and with excess amniotic fluid, there was not much room for the head to remain in that position. The pup needed to be delivered now.

"Are you all right?" Sesshomaru asked with concerned. With a frail nod from his mate, Sesshomaru continued, "Good! I need you to push on the next contrac-"

"Grrraaaaah!!!"

The contraction slammed into Inuyasha before Sesshomaru could finish his sentence. Sesshomaru felt the pup's head was in contact with his fist. Slowly extracting his fist as the head pressed onto the cervix.

Inuyasha could feel that the cervix was giving way for the head to pass. Although the cervix might have been stretched by Sesshomaru's fist but it was no way near the size of the head.

"Love, I need you to push." Inuyasha heard Sesshomaru strained voice and concentrated with all his might and pushed.

"Keep going!" Sesshomaru encouraged. He positioned himself in-between Inuyasha's widespread legs, with his forearm still inside Inuyasha.

"Ahhhh..." Inuyasha yelped in pain as he felt a sharp tug pulled at his body. He bit his lip and pushed. His head was spinning and blood was rushing to his brain. He wanted the pain to go away but it was just getting worse.

"I can't," Inuyasha panted heavily as tears fell from his eyes, "I can't do this... It hurts too much."

"You can do it. Push harder!!" Sesshomaru ordered from between his legs.

Inuyasha's body pulsated with fiery pain. Another raspy and scratchy scream escaped his throat. Sweat glistening on his fair skin and his body was on fire. Crying hoarsely, he dug his chin into his chest and pushed again.

After a few contractions and pushing, the head still failed to exit from the uterus. This was one of Sesshomaru's worst fears; the head was bigger than what the cervix allowed.

Energy was draining out from Inuyasha and he was getting worn out. If he passed out from exhaustion, he was not only putting the pup in danger but also himself. Sesshomaru might not be able to pull the pup from the inside but he could help his mate by pushing it from the outside.

On the next contraction, Sesshomaru pressed non-too-gently onto Inuyasha's bulging abdomen. Inuyasha squeezed his eyes shut and tried to breathe. However, the pain from both the contractions and Sesshomaru’s pressing caused him to wail out loud.

"UAAGGGGGHHHH...!!!"

Inuyasha cried harder when he felt something was tearing him from the inside. Something warm oozing out and he did not like the feeling. His body was quivering badly and his fingertips were bleeding from burrowing holes into the soil.

When Sesshomaru felt that the head had emerged from the uterus, he slowly slipped out his hand. To his horror, his entire forearm was shiningly coated with Inuyasha's blood and fluids trickling to the ground.

'Why is he bleeding so much?' Sesshomaru questioned himself.

"Inuyasha?" Sesshomaru gazed at his mate worriedly.

"Sess...Sesshy..." Inuyasha tried to speak but words seemed beyond his reach as he was trembling and tweaking in pain.

Inuyasha felt ill to his stomach and the unbearable pain was searing hot through his body, numbing his senses and making him ache. A vast patch of grass between his legs was dyed with crimson fluids that was gushing out from his stretched opening, warming up his nether region.

"Sesshy!!" Inuyasha roared when another severe pain racked through his abdomen. He felt something forced its way downward, gliding toward his opening. His body seized uncontrollably from the immense pain and the world spun before his eyes.

In an instant, Sesshomaru held the insides of Inuyasha's thighs up and out. A patch of silver hair could be seen at his entrance.

"I can see the head!" Sesshomaru exclaimed, "Keep going!"

Inuyasha exhaled and sucked in more air with his mouth. He bore down again and pushed fiercely. Black spots danced at the back of his eyelids as he repeated his actions.

Giving another big push, he screamed out in excruciating pain as his breath hitched in his throat when his small opening was violently split open and something big slide out.

The head is out!
